Street Medicine Coordinator
(OPS Human Services Analyst)
This position serves as the coordinator and provides eligibility
assistance and support services for new street medicine initiative
of the Health Care for the Homeless Program that will provide
comprehensive primary medical, dental, and case management services
to street homeless individuals. This position entails engaging with
other health and social service provides as well as going out to
the streets and into encampments to engage with the homeless
population.
Your Specific Responsibilities:
+ Coordinate street medicine team locations, schedule, and
services
+ Ensure communication among team members, both internal and from
partner organizations
+ Facilitate team huddles
+ Coordinate ordering of medical, office and other supplies
+ Coordinate pickup and delivery of supplies
+ Drive medical van, ensuring arrival to and from sites as
scheduled
+ Coordinate cleaning, maintenance, and fueling of van and
equipment
+ Assist clients with navigation of services and resources
+ Conduct eligibility determination and enrollment into services
based on established criteria
+ Confirm and updates client demographics
+ Coordinate patient appointments
+ Monitor patient referrals
+ Provide information on and refer individuals to public or private
agencies and community services for assistance
+ Keep updated on community resources for individuals experiencing
homelessness
+ Keep records and prepare reports as required
+ Maintain client confidentiality in accordance with Florida
statutes
+ Demonstrate good interpersonal communication skills via in-person
and/or telephone encounters
+ Use multiple computer applications to perform duties and keeps
technical skills current which may include: NextGen, CareScope,
HMS, Eaglesoft, and/or Change Healthcare
+ Adhere to established safety procedures and practices
+ Ensure medical record releases are completed and filed
appropriately
+ Answer all telephone calls courteously and promptly with accurate
information
+ Maintain resources for client information
+ Attend team and other meetings as assigned
+ Keeps supervisor and team members informed
+ Perform other related duties as assigned including emergency
duty
Required Knowledge, Skills, and Abilities:
+ Ability to plan, organize and coordinate work assignments
+ Ability to use independent judgment and decision-making
+ Ability to work independently and manage one's own time and the
time of others
+ Ability to use critical thinking
+ Knowledge of customer service principles
+ Ability to communicate effectively both verbally and in
writing
+ Ability to develop and maintain effective working
relationships
+ Skill in computer software programs used to facilitate job
duties
+ Ability to follow instructions
+ Knowledge of medical terminology
Qualifications:
Minimum-
+ Valid Florida Driver License and access to reliable
transportation
Preferred-
+ 1 year or more of program coordination experience
+ Experience working in a medical clinic environment, including
knowledge of medical terminology
+ Three or more years of customer service experience
+ Three or more years of data entry experience
+ Three or more years of experience determining financial
eligibility for services
+ Experience working with the homeless population
+ Experience providing services in an outreach setting
