POSITION SUMMARY:

Full Time employee needed to maintain an active caseload consisting of individuals with mental health or co-occurring disorders in Pinellas County. Specialists will work directly with clients in the community to assist them with locating and transitioning into independent living. Transitional Housing Specialist will link them with the necessary community support services, prepare assessments and any related social, financial, or medical history reports or forms as they pertain to the clients needs. Specialists will be primarily responsible for the clients and accountable for the coordination, management, and provision of quality services to ensure the clients welfare.

WHAT YOU WILL DO:

  • Prepare assessments and any related social, financial, or medical history reports or forms as they pertain to the clients needs and case record.
  • Establish a relationship with the client, family/guardian, significant other, and all other service providers connected to the client.
  • Participate in client staffing's and follow-up as assigned.
  • Coordinate the service planning process, with the primary goal of assisting the client with achieving independent living.
  • Apply for support funds as needed.
  • Maintain an active caseload consisting of clients in the community and link these clients with the necessary community support services required for maintaining the individual.

WHAT YOU NEED:

BS/BA degree in the human services field. Experience in behavioral health preferred.

Must participate in at least 10 hours of continuing education/training per year.

Must be computer literate.
