Job Description:

The Case Management Assistant - Hospital, under the direction of Care Coordination department, assist with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care; responsible for assisting with distributing regulatory documents to identified patients; educating patients about the appeal process related to their discharge; performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service.
