Job Description:
Lutheran Services Florida (LSF) envisions a world where children

seeking a talented Licensed Practical Nurse who wants to make an
impact in the lives of others. Purpose and Impact: The Licensed
Practical Nurse coordinates all physical, mental, and dental health
services for children served through the Case Management
Organization. Assists in the identification, coordination, and
completion of service and evaluation of health or family problems
of enrolled children. Maintains awareness of current health
services available in the community and works with physicians and
other local health care providers to meet the needs of children
served. Responsible for the coordination of health follow-up
services to identified children. Documents all health/family
services that have been provided in the child welfare information
system. Requests, interprets and monitors provider records and
reports and ensures services are provided for children within
required programmatic timeframes. Essential Functions: Reviews,
assesses and evaluates child health records Screens each child for
health status as appropriate Plans and recommends appropriate
actions for identified health problems for children in care. In
accordance with the Nurse Practice Act, delegates to appropriate
staff the administration of medication and skilled nursing
procedures for individual children as appropriate. Monitors the
administration and documentation of the individual children files.
Serves as consultant to appropriate staff regarding individual
child's health needs. Communicates with parents and appropriate
staff regarding health status of children with identified health
concerns. Obtains information from other health and education
providers and incorporates their recommendations into health
service plans for children in care. Provides initial and follow-up
screenings (vision, hearing, height and weight) Documents all
action, completion and/or needs of the child. Maintains cooperative
working relationships with physicians, dentists and other local
health care providers in order to provide optimal healthcare for
enrolled children. Refers case managers/families to appropriate
community services for identified health needs and documents all
such referrals. Develops and monitors tracking of medical, dental,
and psychotropic medications for children in care and ensure timely
entries of updated appointments, medication changes, etc. Works
with case managers and families to obtain appropriate consents for
medication and treatment as needed. Participates in the Case Review
process, as appropriate. Administers first-aid or other emergency
care as necessary while on site during emergencies. Maintains
strict confidentiality at all times, including the ability to
recognize situations in which confidentiality is required. Attends
all workshops and meetings as deemed necessary by the Program
Director/Quality Assurance Manager. Attends all required staff and
meetings. Other related duties as assigned. Other Functions: Note:

activities described above. Physical Requirements: Must be able to
lift at least 45 pounds. May be subject to contact with
communicable diseases; subject to dealing with families from
dysfunctional and/or diverse multicultural backgrounds; subject to
working in cramped or non-existent office space. Must be able to
travel to complete assignments in Lake/Sumter Counties. Education:
LPN required; BSN or RN preferred Experience: Requires two years of
experience in community health, pediatrics or school nursing.
Bilingual preferred: English/Spanish or English and other languages
present in service area. Knowledge, Skills & Abilities: Knowledge
of human development and health. Knowledge of social services and
health services available in the area. Knowledge of computer data
entry processes. Knowledge of family-centered practices. Ability to
communicate ideas and instructions orally and in writing. Ability
to write reports and business correspondence. Ability to speak
before groups of individuals, such as Committees and Staff
meetings. Knowledge of human development and health. Knowledge of
social services and health services available in the area.

family-centered practices. Ability to communicate ideas and
instructions orally and in writing. Ability to write reports and
business correspondence. Exceptional organization skills and
attention to detail are required and the candidate must be able to
meet designated deadlines. Other: Must demonstrate sensitivity to
our service population's cultural and socioeconomic characteristics
