Full-Time Home Health Physical Therapist to join our team. This
role focuses on helping patients achieve their maximum levels of
function and independence in their homes. The successful candidate
will work to develop, recover, or maintain the daily living and
work skills of people with physical, mental, or developmental
conditions. This position offers a unique opportunity to make a
significant impact on the lives of your patients, providing
personalized and compassionate care tailored to their individual
health needs. Conduct patient assessments and evaluations to
determine their physical condition, limitations, medical histories,
and personal goals. Create tailored treatment plans that address
patients' specific needs and goals. Implement therapeutic
interventions for pain management, motor function, and overall
mobility enhancement. Educate patients and families on proper home
exercise programs to ensure continuous treatment progress.
Collaborate with other healthcare professionals to ensure optimal
patient care and insurance documentation as required. Strong
interpersonal and communication skills Expertise in various
physical therapy techniques Ability to work independently Knowledge
of proper exercise and manual therapy techniques Medical Insurance
Dental Insurance Vision Insurance 401k Valid Physical Therapy
license in the state of practice Valid Driver’s License and
reliable transport CPR certified
