Advanced Care Center - Purpose of Your Job Position
The primary purpose of your position is to coordinate and
facilitate referrals for potential admissions. The Admissions
Director will act as a liaison to the nursing unit to help ensure a
smooth transition with the admissions process for all residents.
Further, the position will act as a liaison with family members as
well as the residents of the Facility.
Delegation of Authority
AsAdmissions Directoryou are delegated the administrative
authority, responsibility, and accountability necessary for
carrying out your assigned duties.
Job Function
Every effort has been made to identify the essential functions of

Duties and Responsibilities
Administrative Functions
- Maintain a daily log of referrals for bed vacancies. Share log
with screener and Administrator.
- Follow fax and phone procedures regarding bed vacancies to area
discharge planners.
- Obtain residents insurance information when possible at the
time a referral is called in. If insurance information is not
available, obtain insurance information from screener
immediately.
- Admit resident to Facility by requiring resident or responsible
family member to sign admissions packet within forty-eight (48)
business hours.
- Advise residents family of co-pay amounts and collect revenue.
Provide revenue to the Business Office.
- Maintain an admissions filing system as devised by the
Admissions Director/Customer Services Director. Assist in
developing procedures to ensure records are properly completed
before filing.
- Responsible for communicating insurance approval within thirty
(30) minutes to the screener as soon as it is received to expedite
admission of potential resident(s).
- Stay in constant communication with all screeners throughout
the day.
- Relay admissions information quickly and efficiently to the
Admissions Director/Customer Services Director, Administrator, and
all other departments, if necessary.
- Assist potential residents and their families by conducting
tours of the Facility and answer questions regarding the Facility
and admissions process. Provide residents with admission
information packet (e.g., resident rights, notice of privacy
practices, admissions contract, etc.). Review as
necessary.
- At the time of admission, meet with the resident and their
families to assure all necessary forms are completed. Provide forms
to the Business Office.
- Obtain the following information from all potential residents:
PRI, complete admission application, bank statements, financial
support statements, and proper insurance verification.
- Assure that all departments receive necessary information to
prepare for admission (i.e. medications, treatments, and any
special needs).
- Switch residents from HMO managed care organizations to
Medicare.
- Receive and follow work schedules or instructions from your
supervisor and as outlined in our established policies and
procedures.
- Develop and maintain a good working rapport with
inter-department personnel, as well as other departments within the
Facility.
- Assure that work areas are clean and records, files, etc., are
properly stored before leaving such areas on breaks, end of work
day, etc.
- Forward completed medical records to the appropriate
department, if necessary.
- Notify nursing service when the resident has arrived at the
Facility to assure that a member of the nursing staff escorts the
resident to their assigned room.
- Review and explain the resident, and/or guardian, room rates,
billing procedures, visitors and guest privileges, restrictions,
resident care procedures, etc., as appropriate.
- Keep abreast of current Medicare and Medicaid regulations
governing admission and discharge requirements of health care
facilities.
Committee Functions
Serve on various committees of the Facility, as directed.
Attend subacute meetings to review financial status of short-term
residents.
Staff Development
Attend and participate in Facility mandatory in-service training
programs as scheduled (e.g., OSHA, TB, HIPAA, Abuse Prevention,
etc.).
Attend and participate in workshops, seminars, etc., as
approved.
Resident Rights
- Review complaints and grievances made by the resident and make
written and oral reports to the Administrator. Follow Facilitys
established procedures.
- Maintain a written record of the residents complaints and/or
grievances that indicates the action taken to resolve the complaint
and the current status of the complaint.
- Must adhere to all HIPAA requirements.
Working Conditions
- Works in office areas as well as throughout the
Facility.
- Attends and participates in continuing educational
programs.
- Is involved with residents, family members, personnel,
visitors, government agencies and personnel, etc.
- Communicates with the medical staff, nursing personnel, and
other department directors.
- Maintains a liaison with the residents, their families, support
personnel, etc., to assure that the residents needs are continually
met.
Education
Must possess, as a minimum, a high school diploma or GED
equivalent.
Experience
Two (2) years prior admissions experience in health care facilities
preferred. Ability to type is required.
Specific Requirements
- Must be able to read, write, speak, and understand the English
language.
- Knowledgeable in Medicare requirements and insurance providers
i.e. HMO rules, Medicare rules, RUGS II & III, and Medicare
prescription plans.
- Must possess the ability to make independent decisions when
circumstances warrant such action.
- Must possess the ability to deal tactfully with personnel,
residents, visitors and the general public.
- Must possess the ability to minimize waste of supplies, misuse
of equipment, etc.
- Must possess the ability to seek out new methods and principles
and be willing to incorporate them into existing admission
practices.
- Must possess leadership and supervisory ability and the
willingness to work harmoniously with and supervise other
- personnel.
- Must be able to evaluate the performance, initiate disciplinary
actions, and prepare and complete performance evaluations for
personnel.
- Be knowledgeable in the use of computers (Excel, Word, and
Access), data input, retrieval and output functions, etc.
- Must have patience, tact, cheerful disposition, and enthusiasm,
as well as must be willing to handle residents, staff, and
visitors, based on whatever maturity level at which they are
currently functioning.
Physical and Sensory Requirements
(With or Without a Reasonable Accommodation)
- Must be able to move intermittently throughout the
workday.
- Must possess sight and hearing senses, or use prosthetics that
will enable these senses to function adequately so that the
requirements of the position can be fully met.
- Must meet the general health requirements set forth by the
policies of the Facility, which may include a medical and physical
examination.
- Based on the Occupational Safety and Health Administration's
Guidelines for Nursing Homes Ergonomics for the Prevention of
Musculoskeletal Disorders and the American Conference Governmental
Industrial Hygienists' Threshold Limit Values for Lifting the
Facility has identified that this job may require the lifting of
residents, equipment, or other objects. Accordingly, this job may
require a minimum of 5 pounds and a maximum of 25 pounds lifting,
periodically and or as needed.
